I told you not to tell!
I think the answer is "slowly"-- which has the virtue that everyone can "play" in little five or ten minute increments when they're in front of a computer, instead of blocking out multihour sessions of time that our significant others reasonably think we should spend with them.

I agree.
Starting at 3rd or 4th level also allows players to put together a package of skills, feats, equipment, multiclassing if desired, and one or two magic items that add up to a more distinctive and coherent character concept than a 1st-level character. The PC creation software that comes with the player's handbook makes this much easier than it used to be (e.g. tell everyone that they can use the gold allotment that the software gives them, and buy equipment including magic items from it).

To start a conversation: who likes starting a game at 1st level? Who likes some other starting point? What? 3rd? 5th? 7th?
